Training Verification

The Department of Obstetrics and Gynaecology at the University of Toronto will provide verification of postgraduate clinical (residency or fellowship) training that took place in our department. We charge an administrative fee for this service, according to the schedule below.

Please note that we CANNOT provide verification of undergraduate medical training at the University of Toronto, nor can we verify MD degrees. Requests for confirmation of University of Toronto degrees can be fulfilled at degreeconfirmation.utoronto.ca. Further questions about undergraduate medical education should be directed to the MD Program.

Please also note that we are unable to provide verification of training that took place before 1976, as registration for postgraduate training at that time was overseen directly by hospitals rather than by the University department. Requests for training verification from before 1976 must therefore be directed to the individual hospital's credentialing office. A list of affiliated hospitals can be found here.

 

How to request a training verification

Requests for training verifications should be sent via email to obgyn@utoronto.ca with the subject line "Training verification request".

Requests should include:

  • any required forms, attached to the email
  • all relevant information about the trainee in question, including:
    • what program(s) they were enrolled in
    • dates of enrolment
    • date of completion of training
    • former name (if trainee's name has changed since training was completed) 
  • indication of which method of payment will be used to pay the administrative fee (see below)
  • signed evidence that the former trainee in question has consented to have this information released for this purpose

We will ordinarily provide a training verification in PDF format via email. Requests for a physical form or letter (with or without a raised seal) will incur an additional fee as outlined in the schedule below.

Payment by credit card takes place via a link we will send to the requester via email, which redirects to a secure third-party payment system. Do NOT send credit card information directly to University staff. 

Payment by corporate/institutional cheque, certified cheque, or money order should be made out to "University of Toronto" and mailed to the address below. Cheques and money orders must be drawn in either Canadian dollars, or in U.S. dollars (according to a conversion provided by our office). We are unable to accept personal cheques, cash, or United States Postal Service money orders. If you require an invoice in order to issue a cheque, please indicate this in your verification request email.

Please note that payment of the administrative fee according to the schedule below must be completed before any verification will take place.

While we will make every effort to expedite verification requests, please note that it may take us up to ten business days to complete requests, especially if a search through non-digitized archives is required. In addition, due to the covid-19 pandemic, all staff are working remotely, and so accessing physical archives may take more time than normal.
